How To Prepare For Lactation Visit

I’m really glad you chose to schedule a Lactation Consultation with me. I just want to go over a few important details before our visit:

• My fees are paid at time of visit- I accept cash, check, Mastercard, Visa, Discover Card, and American Express.
• After the visit, I will provide you with a Superbill that you may submit to your insurance company for reimbursement for my services. I cannot promise they will do so. Refunds for consults are NOT given under any circumstances!!

Your preparation checklist

• Write down any questions you may have for me.
• Prior to the visit, I’ll send you some forms including  a consent form, and a health history. Please fill these out beforehand. This will be helpful and allow us to spend quality time on consult.
• If you are using a breast pump or nipple shield, please have them nearby for demonstration of assembly and proper use. 
• If you have any pets, particularly dogs, please put them in another room. Animals can become very possessive and territorial  with babies and I don’t want anyone to get bit!
• Please have  baby with you for oral exam, latch assessment, pre-feed and post-feed weights, and any other concerns I can address.

After the visit checklist

• I will provide you a care plan and advise you how to proceed.
• I will fax a written report to your child’s Pediatrician, OB/GYN, and/or other health care providers. You will get a copy as well.
